++By online registering to attend the "Can't Fake The Shake"& I-Motion Fundamental Basketball Camp, I (as the parent/guardian being of legal age of 18 or older) agree and acknowledge that I voluntarily have chosen to allow my child to participate in a program of progressive physical exercise. By submitting this form, I (as the parent or guardian) assume all risk to my child's health and well being and any resultant injury or mishap that may affect my child's well-being or health in any way. I (as the parent/gaurdian) "hold harmless" of any responsibility, the instructors, the facility, any persons or entity involved with the "Can't Fake The Shake"  and I-Motion Fundamental Basketball camp.
